Minimising your incisions and scars
For any woman considering breast enlargement, the obvious attraction is achieving a beautiful natural shape and a feeling of confidence in her body. As well as looking great in lingerie, a cocktail dress and a bikini, it is also important to be confident naked! This means that any scar resulting from surgery should be small, discrete and as “invisible” as possible.

No matter what technique is used, some form of scar is inevitable. The size, position and quality of the scar are determined by many different factors. There are a number of options available for placing the incision, including:

  • The infra-mammary scar – The commonest technique, involving an incision at the base of the breast. The horizontal incision is usually about 5cm in length.
  • The peri-areolar scar – This incision has become much more popular in recent years. It is contained within the lower half of the darker skin of the areola (the pink or brown circular area around the nipple)
  • The axillary (armpit) scar – This approach places a scar in a natural crease in the skin within the armpit area. In most cases, the scar heals very well and has the obvious advantage of avoiding any scar on the breast.
  • The peri-umbilical (belly button) incision – A scar around the belly button for breast enlargement has mostly been used in the United States. It has the advantages of avoiding any scar on the breast but has some significant “downsides”.

I was wondering if there is an alternative to a full breast lift with augmentation as i've read about donut lifts and feel that the full lift would be extreme given my age and degree of lift needed.

Hi, I need some advice and hope you can help. I had breast cancer two years ago and following radiotherapy devloped lyphoedema. I have very large breasts 30/32 G cup. I have received treatment for my lymphoedema and am told by my therapist that things are as good as they will get. I am also considering a breast reduction, but have been told by the surgeon that because of my lympoedema that he would only be able to offer me the "Robertson Technique". This method involves horizontal scars and the limited information I have been able to find suggests that it is not the best method for breast reduction. Having my breasts reduced is very important to me as I am very out of proportion being a size 8 at 5ft 6. I would apprecaite advice on where I can find out more about the Robertson technique and if there are any before and after images. Also if you think this method of breast reduction is the only option for me given my circumstances. I really apprecaite any advice you can offer. Many thanks

hello, I am 19 years old (almost 20) and am thinking about having breast augmentation. I'm 5ft 8" and around 12 stone, I am very uncomfortable with the shape of my breasts and feel surgery is my best option for the result I am after. I'm looking for larger breasts - from 38B to 38C and would like a lift aswell, as I feel my nipples are far to low. I'm probably not going to go through with it for a few years yet, as I feel I am still a little to young, but I am starting my research now and you guys seem great! Is there any way you could give me a rough price so I know how much to put by? Thank you for your time :)

Hi, i've been looking 2 have my breast enlarged 4 some time as i've always felt a bit behind in this department!! i'm a small 32B sometimes a A, my skin tone is white british but i tan well so have more of an olive skin type, i've had a small fatty lump removed from my right boob but and they cut around the bottom of the nipple and the scar is very faint, i'm slender build dress size about a 8/10 and my weight is about 9st. please advise me on what size you think is best as i'd like 2 be a full C/D but DON'T want 2 look fake, and whats the best look under or over the muscle?? look forward to reading your reply!! many thanks, Zara.

Hi, I had an uplift and implants 2 years ago. When i came round from my surgery the surgeon had put the implants under my muscle but i had clearly asked for them to go over as i wanted that look. I then had 3 children and had saved to have my dream boobs, but they have been a nightmare. One is larger than the other, one of my nipples has a square edge where they were not shaped properly and the skin is really thin on my right breast on the underneath. To top this all off my scarring looks as though i have been butchered. I get tremendous pain in my right breast, i am now looking to have another uplift and smaller implants to make my breasts look normal. I just really want a little advice. thanks

I am 5.1 and have two children which I have breastfed. Before pregnancy I was a 30E. However my breasts are now 32GG and they are so saggy and heavy and the nipple has drooped too. I am very conscious of my body now as they are just too big for my frame and look hideous. I do not even like my partner to see me naked any more, and I can no longer see my waist or my stomach. This is causing me great distress. I would love a reduction and an uplift and not sure whether to go for a B or C cup. i had a breast augmentation twice through the NHS about 4yrs ago taking me to a C/D, the 2nd time was due to the implant in my right breast not looking the same size as the left after op, also the right breast scar looks more noticable than the left. since then i have had a baby, so as usual this has affected my shape and size. i am looking to go private and get them done again to EE and hopefully get a surgeon who would minmise the scars aswell. do you have a rough idea of this costing and the type of procedure. many thanks Tasha

hi i am hoping to get my surgery done in around 2wks time, i am of west indian desendent, i am hoping to get just implants, but have been told by a few surgeons that i need a uplift, but the crazy thing about it is they only have to move my nipple up a fraction, bascailly the move is the size a your little finger nail, which i stated that its not worth paying unecessary money to move my nipple up, because my nipple has never been right in the middle, im hoping for a more natural look so im opting for under the breast but some say no it has to be ontop of breast and some are saying underneath,im not sure what size to go as i am a size 36b/c i have tried my sisters bra and she is a 38e/dd depending on make, will that be too big, my breast is very nice but lacks a little volume and when i lay down it wrinkles so there is enough room to go to maybe 2 sizes what do you think,i have to decide as i need to get them done, soon as ive already book my holiday of work.

Hi i had breast implants just over 5years ago. Everything seemed fine for the first few months, but then noticed that the right breast was considerably harder than the right. I phoned the hospital to arrange to see my surgeon, but could not get an appointment for 2 months. During this time i found out that i had started my menopause, which got me thinking that, that could be the cause due to the changes my body was going through. I eventually saw my surgeon, who assured me it was'nt the menopause, and that i had developed thick scaring around the implant. I needed it done again. Which was going to cost me. I do not wish to go back to the same surgeon. Can you help me?

Hi I recently had a consultation with Mr Liew and he suggested I have over the muscle. I am currently a 34B. I do need do loose a bit of weight and my boobs are smaller when I loose weight so concerned they will look unnatural if I loose weight. Should I just have under the muscle? He said they can look far apart if under the muscle but I have seen lots of photo's and they look really natural on sme people and not far apart therefore are some surgeons just better at it than others? Surely this is the best way if less chance of capsular contraction. I also asked about any other incision other than under the breast but he just sort of dismissed the idea. Once again I have seen photo's of nipple and armpit incision and they look great. Do any of your surgeons offer this or are there disadvantages to these incisions. Surely if the surgeon is good how can an armpit incision have disadvantages - maybe there is, but I don't know what they are. It seems we are a bit behind (in the uk) when it comes to doing these types of incisions as all the photo's of breast implants in America seem to be umbilical, armpit or nipple and they are not all saline implants. Hi I have grade 3 capsular contractuar in my left breast it began 2yrs ago 6months after having silicone implants which were placed under the muscle. My surgery was done in the US fear has stopped me doing anything about this however my breast is solid and mis-shapen. Do you do corrective surgery and what would it involve? After so long would it mean having to remove damaged breast tissue? Would both breast implants have to be removed? Having had a baby and breastfed I went from a 30D cup to a F cup, and back again, leaving the breasts droopy and emptier looking, lacking their former pert appearance. They look just fine in a good bra, but as you said, it is important to look good naked! I am not sure if 'filling them out' with an enlargement would be sufficient, thus returning them to a fuller shape? Or would I need the uplift to get the right result? I feel less sure about the uplift as it appears to be a lot more involved, and ofcourse the scarring is an issue. How ever I do have a surgical scar from my c.section that is almost invisible, thus I suppose I could have the potential to scar well with breast surgery also? I Would really appreciate your advice, many thanks, Laura.

However, this is only when I wear a really good bras; when I take them off my boobs disappear and my nipples are virtually pointing to the floor. I think I need an uplift and implants but I'm worried about what size I'd be afterwards. Having researched this, I think round implants would be better as I have lost all the volume from the top, but I also don't want them to look fake, just full and shapely. I haven't had children and although I think I will eventually get round to it (I am still single, mind!) I am so unhappy with how my breasts look, I can't wait until I've finished having children before I have this surgery. On top of this I also have a little more weight to lose, so is it best to get down to my ideal weight before having surgery?

Hi - I had breast implants about 5 years ago and had the nipples re-positioned and the implants put in through those incisions. However my suurgeon put in much bigger implants than I wanted so i have never been happy with my breasts which have now slipped off the implant so I am looking into having them removed but will need a mastoplexy - is there a way of doing this without the large 't'shaped scar. I also want the nipples made smaller as the area is so big following the original surgery. I don't have sufficient breast tissue to simply have the implants removed I would need a lift and the excess skin removed. What do you suggest and are there surgeons who specialise in this proceedure? If not how do I find one who will do a really brilliant job for me?

Hello, I'm going to be having mastopexy Agumentation as I have size B breast that have become very flat and saggy due to weight loss, I am hoping to Increase to at least a D cup. My surgon said he could do this without the lollipop or anchor like scaring as I have hypertrophic scaring, I am aware that there still will be scaring involved and you talk above about scare reduction treatment, are there any you can make me aware of? Dear Norman, I have 2 child just over two years old and whilst I was pregnant I put around 2 1/2 stone on. Since having my son I have lost 3 1/2 stone, weighing in at just over 9 stone. I'm happy to have shifted the baby punds but my breast have shrunk to a 34A and don't seem to feel "meaty" anymore. I would like to increase the size of my bust, but only by 1 or 2 cup sizes. Yes , you are a very petite build and this may have a bearing on the appropriate size for you. The most important thing is the dimension of your existing breast and in particular, how wide it is at the base. This will determine the size of an implant which will fit you to give a full breasted but natural look. If you go bigger than the size of your body shape, the implants will be very obvious and over round. At any consultation, your breast measurements will be taken and you will have the opportunity to try out the size in a bra. For small girls who wish to have fuller breasts, there is also the option of choosing a 'high profile' implant which gives more projection.

Being black or mixed race does not automatically mean you will get thicker scars but you are right that there is a slightly higher risk. If you have made good scars from previous surgery such as an appendix removal, it is likely you will make good scars from a breast augmentation. It is also worth knowing if you or your family have made bad scars as the risk may run in families. The decision as to where to place the scar has to be made after a clinical examination. If you have a reasonably sized nipple and areola ( the darker skin around the nipple), it may be an option to place the scar in this area. For all patients undergoing breast augmentation, there are a number of things that can be done to soften and mprove the scars after surgery. These include the use of a silicone remodelling cream ( see www.healgel.com ). You will be advised in all apects of scar management after your surgery.

Dear Laura, It sounds as though your breasts have drooped as well as reduced in size after breastfeeding. If the nipples are now quite low on the breast , it is often not a good idea to just have a breast augmentation. Although it will restore the size, the nipples will not be in the right place and the shape is not attractive. You are right that an uplift does involve more scars. Essentially it is a trade off between SCARS but with a better firmer more attractive breast shape. Although it is encouraging that you have produced a good scar from your Caesarian Section, it doesn't always mean that the breast scars will be as good. However, for most women, the scars fade a lot after a year.

Dear Gail, It sounds as though your breasts have dropped as well as become smaller after having your children. This means you probably will need a lift as well as implants to restore your size and a better shape to the breasts. This is called a mastopexy augmentation. It does mean that you will have more scars than with just an augmentation. The scars usually go around the areola ( the darker skin around the nipple) and another scar leading from the middle of the breast under the nipple down to the base of the breast. There may also be a scar transversely in the breast crease where the breast meets the chest wall. You can get some information and pictures from the website. Dear Saira, From the sound of your symptoms, it is likely that you need a breast reduction rather than liposuction to the breasts. Your problems of sore shoulders and neck and back pain are very common for girls with very heavy breasts and is a frequent reason for seeking breast reduction. We do have female consultants at MyBreast although I do not know what area of the country you are from. It may be that we don't have any in your region. The staff at MyBreast would be able to advise you if you give them a call.

'As you are only four weeks after your surgery, don't worry about minor dog ears at this stage. Sometimes, they persist and may require a small revision procedure under local anaesthetic after six month's or so. As far as managing the scars is concerned, once the wound is healed, you should be advised on a program of moisturising and massaging the scars. After six weeks, it can be very useful to apply a silicone gel to the scars which may speed up the maturation process and stop the scars becoming lumpy.'

Dear Alice, Arm lift is an operation that does leave scars, which can be difficult to hide. The scar extends from the inner elbow up to the axilla ( armpit). The risk of hypertrophic or keloid scarring is increased for black patients although it is not inevitable. It is helpful to know if you have other scars and whether they healed well. Ideally, you should try to lose some weight before considering surgery.

Dear Becky, You have raised a number of issues. Firstly the size of any breast implants is determined by the shape and size of your breasts, in particular , the width of the breast. In this way, an experienced surgeon will be able to advise you on choosing an appropriate size that will fit your body for the most natural result. There are many views on whether to choose round or tear drop breasts and both have advantages and disadvantages. It is possible to place the implants through an incision around the nipple as long as the areolar is large enough. Again, there are often individual issues which determine the best approach. It is possible to breast feed after breast enlargement. Many women have implants and further children afterwards. You wil get a clearer idea of what options may suit you at a consultation with an experienced surgeon.

Dear Aurora, It sounds as though you would definitely need a mastopexy ( uplift). It may be that you dont need implants but this depends on how much breast tissue you have.If you are currently an F cup, I suspect you wont need implants. A lift would reshape your breasts and restore fullness to the upper part. It is definitely worth losing weight before surgery. If you have surgery and lose a lot of weight, it can affect the appearance of the breasts.

Dear Alyssa, If you have an uplift , you will have a smaller cup size, but it should be around the same volume as you are now( DD). This is because the tissue is reshaped into a more youthful position by an uplift but no tissue is removed. You can see some examples of this procedure and photographs on the website.

Dear Rachel, Having a breast enlargement is a serious decision and you need to be sure that this is right for you. Without an examination, it is not possible to say whether the implant should be placed above or below the muscle. if the implant does go behind the muscle, the recovery period is slightly longer. The position and size of the scars depends on a number of factors and again this is best discussed at consultation with a surgeon.

Dear Jane, A breast reduction or uplift does involve scars and as you already know from your gall bladder operation, you have a tendency to form hypertrophic ( thick) scars. There are some treatments to minimise or reduce scars, but if you really don't want any scars on the breasts, I am afraid you probably shouldn't have surgery. The only option possible would be to increase the breast size using fat transfer but this is a difficult procedure and is not suitable for everyone. indeed, from your description of having heavy droopy breasts, it really wouldn't work in your case.

One thing I should say is that it is absolutely possible to have natural looking breasts with round implants. The important thing is to choose the right size of implant to fit your own breasts.

Dear Sue, I am afraid my advice is the same I gave to Jane. Correction of tuberose breasts does involve scars, usually around the nipple. if you produce keloid scars, surgery may not be for you. There is the possibility of augmenting the breast using fat transfer but this won't usually work well for tuberose breasts.

Dear Lucy, The decision as to size and shape of the implants can only really be determined after your breasts have been examined and measured. Implant size and position depends on the width of your breasts, how much tissue you already have and the position of the nipple. All of these issues would be thoroughly discussed at a consultaton to determine the best option for each individual.
